Realism to a point is a good thing, everyone prefers different things. I think if you focus on the roleplaying aspect, then realism may be more necessary - such as having sheathed swords and peace strings (otherwise, you would get attacked by the guards in town). If you focus on hack and slash, realism isn't that important - kills are. If you focus on a theme, staying true to theme >is< important. Add as much (un)realism as necessary to achieve an enjoyable theme. Everyone has their preferences.
